VisionQuest to Assisi Traveler Forms & Information

Traveler information: Must be completed separately for each passenger.

Third party credit card form: gives Lore Raymond signed permission to charge passenger credit cards on the dates and for amounts noted in the other documents.
 

These documents are provided for passenger information and use as needed:

Parent permission form:  Parents are no longer permitted to travel out of the U.S. solo with a child unless they have obtained the other parent’s notarized agreement.  This form is for the passenger’s personal use.
 

Insurance.pdf:  this document explains the difference between the two plans offered for $131 and $164 per person, respectively.  It will assist passengers in making their insurance selection on the Traveler information form.
 

Due dates:  Note that all passenger documents along with $440 deposit and 100% of the the travel protection insurance fee are due in December.
